 Parasites in 3-toed box question

I’m wondering if anyone could offer some advice. I have a 3-toed box turtle (~3 year old) that has been straining to go to the bathroom and pushing his insides out. I think it may be his "male organs" but am not sure. Read that they may do this at times during their lives but may be due to a parasite problem. I’ve asked my vet about this and he gave me panacur to give to him. The first round didn’t seem to stop him from doing this but it is extremely hard to give him oral meds. I’ve been using a syringe with a catheter like extension to wiggle it into his mouth and dispense the meds. Problem is then he closes his mouth and the meds come out the corners of his mouth.

Any suggestions as to what is the most likely cause of him pushing his insides out? And if you think this is due to high parasitic levels, any suggestions as to how to more efficiently administer meds?

OR.. is it completely normal to be "fanning" his penis out like this? He doesn’t always do it in the water though. He sometimes does it on his substrate (cedar mulch) which scares me because it sometimes sticks to it...
